Steps to Enroll To finalize your enrollment, we need the following forms completed and returned to your Center Director.
Print from Handbook All required forms are included at the back of the Parent Handbook PDF listed above. Please print and complete Pages 12–15:

How to Submit
Ready to turn them in? You can bring your completed forms with you when you come for a tour.
Note: A valid Immunization Certificate is also required prior to your child’s first day of enrollment.
